Softball Registration Forms
To complete forms prior to registration:

*Once you go to the handouts page you will need to complete the following:
CYSA Registration form, Chesterfield County Code of Conduct form, and Medical Release Form.
Bring all signed forms to in-person registration, an MYA registration form will be provided in-person.
*Please do not forget to bring a copy of players birth certificate for us to keep on file.
Payment is due at registration and can be paid by check, cash, or CashApp $MatoacaIndians
Spring Reg Fees
(includes jersey & visor)
$135: 8u (5-8) Must have turned 5 by 12/31/2023
$160: 10u, 12u, and 15u
Fall Reg Fees
$75: all ages needing a jersey and visor
$50: all ages, return players not needing a jersey/visor
*For any parents/family members/etc to be able to coach/assistant coach or volunteer the
CYSA Sexual Abuse/Prevention form must be completed &
are required to apply for a coach’s card through Chesterfield County.

We ask you to have this done ASAP!! Any coach/volunteer without a coach’s card will not be allowed to participate.
